Quick answer: if the tiny white bump is rock-hard, sits near your eyes, and never comes to a head, it's likely a milium — a small keratin cyst under the skin. If it's a soft, flesh-colored bump on your forehead, chin, or jaw that sometimes turns into a real pimple, it's a closed comedone — a clogged pore. They look similar, but they need opposite treatments, which is why so many routines fail at both.

The 10-second identification test

Feel first, then check location. A milium is firm like a tiny pearl, usually 1–2 mm, and most often appears around the eyes, on the eyelids, or on the cheekbones. It won't pop no matter how you press, because it isn't inside a pore at all — it's trapped keratin sealed under a thin layer of skin. A closed comedone is softer, blends into your skin tone, clusters on the forehead, chin, and jawline, and can become inflamed. Why each one forms

Closed comedones form when dead skin cells and sebum block a pore that stays covered by skin. Heavy, occlusive products, sweat left on the skin, and skipped exfoliation all contribute. Milia are different: the keratin gets trapped under the surface with no pore opening, often after skin trauma, sun damage, or rich eye creams that are too heavy for the thin skin around the eyes. That's also why a sudden crop of small bumps after starting a new product deserves a closer look — sometimes it's purging rather than a breakout, and sometimes it's simply the wrong texture of cream.

Treating closed comedones: chemical, not physical

Closed comedones respond well to salicylic acid (BHA), which is oil-soluble and works inside the pore. Scrubbing is the classic mistake: physical friction inflames the bumps and can push them toward becoming real pimples. LHA, a gentler salicylic-acid derivative, is a smart second step for sensitive skin.

Treating milia: patience and retinal

Because milia have no pore opening, acids alone rarely clear them. Dermatologists agree on the honest version: existing milia often need professional extraction with a sterile lancet, and what skincare can do is prevent the next ones. A retinaldehyde (retinal) product speeds up cell turnover so keratin doesn't get trapped, and around the eyes — where milia love to appear — a purpose-made retinal eye serum is safer than spreading a strong face retinoid onto thin skin. What not to do

Don't squeeze milia — there's no opening, so you'll only damage skin and risk scarring. Don't attack closed comedones with scrubs or layer BHA, AHA and retinoids in the same evening; irritated skin produces more oil and clogs faster.
